Eligibility criteria

General practices and health-care professionals

• Member of the Central England PCRN or a Keele Research Network Practice

• At least two GPs willing to undertake the study as per protocol, i.e., act as a control or intervention practice

• Willing, and able, to allow one (or for preference two – to allow for cross cover) of their practice nurses to be trained to deliver the MOAC 2 clinics

• Able to physically accommodate the nurse clinics in the practice

• Uses the EMIS computerised consultation system

• Nurses and GPs consenting to follow up by the MOSAICS study team

• GPs willing to be trained to carry out the MOAC 1 consultations

• Nurses willing to be trained to carry out the MOAC 2 clinics

• Nurses who consent to being observed and audio recorded in MOAC 2 clinics

Inclusion Criteria:

• Males and Females

• 45 years and over

• Registered with a MOSAICS study practice

• Consenting to further contact from the study team and medical record review (consent sought as part of the Patient Population Survey)

Exclusion Criteria:

• Excluded via GP screen of practice list

• Unable to give fully informed consent, e.g., learning difficulties or dementia

• Resident in a care or nursing home

• History of serious disease, e.g., malignancy, terminal illness

• Unable to consult in the general practice

• Flagged as excluded from research in that practice

  2. *All patients registered with MOSAICS practices randomised to the intervention arm of the study had access to the MOAC 1 and MOAC 2 clinics, including those who did not consent to be followed up, as these ran as part of normal clinical care within each intervention practice.
