  2. 2 Urban: Practice location in town > 100,000 inhabitants (> 30,000 in Slovenia).
  3. 3 PrevServ_5 = Score of the number of ‘yes answers’ from the following items (Table 3):
  4. Item 2: Is CVD risk advice (e.g. about modifiable risk factors such as diet and exercise) integrated with the patient medical record system?
  5. Item 7: Does the practice use case finding methods to detect patients with cardiovascular risk factors?
  6. Item 10: Does the practice have an up-to-date directory of prevention activities/organizations available locally (e.g. gyms, walking groups, weight-watchers etc.)?
  7. Item 11: Did all nurses attend ≥ one training/continuing medical education event on CVD within the last 5 years?
  8. Item 13: Does the practice have a procedure for smoking cessation (e.g. Minimal Intervention Strategy)?
