Inclusion criteria

Exclusion criteria

- High- risk defined by risk calculation with recommended tools according to national guidelines, e.g. > 20% CVD event risk as calculated by Framingham

1. Patients with established CVD (including ischemic heart disease, myocardial infarction, angina pectoris, coronary surgery or revascularisation procedures, ischaemic stroke, transient ischemic attack, claudication or peripheral vascular disease)

or

2. Patients with diabetes

- Patients with 3 out of the following 4 risk factors: hypertension, hypercholesterolemia, smoking, men over 60 years of age

3. Terminal illness, cognitive disorders (e.g. dementia), psychiatric diseases (e.g. schizophrenia) and lack of language knowledge
